内容記述 | PURPOSE: The carbon-ion beam radiotherapy (CIRT) has been introduced in our institute. We have treated uterine cervical cancer using CIRT and C-11 methionine (MET) PET was performed for assessment of the cervical tumor metabolism before and after CIRT. We evaluated if C-11 methionine (MET) uptake was early predictor for local recurrence, systemic metastasis and prognosis in patients with uterine cervical cancer treated by CIRT. METHODS AND MATERIALS: MET-PET was performed in histologically confirmed 71 patients with uterine cervical cancer before and after CIRT. There were 45 squamous cell carcinoma and 26 adenocarcinoma. ECAT EXACT HR+ and ECAT EXACT 47 PET scanner (Siemens CTI, Knoxville, TN) was used for PET imaging. MET-PET findings were compared with clinical follow-up result up to 152.4 months (mean: 46.1 months). MET uptake was measured with a semi-quantitative index, tumor to normal tissue ratio (TNR). Pre- and post-therapeutic tumor TNR, its change after CIRT, rate of local recurrence, rate of systemic metastasis and prognosis were entered into Kaplan-Meire analysis. RESULTS: All 71 uterine cervical cancers accumulated MET in the PET imaging. The intensity of the uptake of squamous cell carcinoma has a higher tendency than that of adenocarcinoma (mean TNR 10.2 and 8.6, respectively), although there was no significant difference between them. Pre-therapeutic TNR had statistically significant relationship to local recurrence, systemic metastasis and prognosis (cutoff=8.9, 14.7, 12.6 and p=0.038, 0.024, 0.026, respectably). Post-therapeutic TNR significantly related to recurrence and prognosis (cutoff=7.5, 3.8 and p=0.0016, 0.0024, respectably), and TNR residual ratio after CIRT significantly related to metastasis and prognosis (cutoff=60.8%, 60.8% and p=0.0090, 0.0024, respectably). In squamous cell carcinoma group, pre-therapeutic TNR had statistically significant relationship to metastasis and prognosis. In adenocarcinoma group, pre-therapeutic TNR significantly related only to recurrence. CONCLUSIONS: MET-PET was a successful predictor for local recurrence, systemic metastasis and prognosis in patients with uterine cervical cancer treated by CIRT. But the relationship depended on the histological type of cervical cancer. | |||||
