This method is based on the theory that both GAG and Gd-DTPA are negatively charged, that is, Gd-DTPA will distribute in cartilage inversely to the concentration of GAG.\n\\n(Methods)\nNine knees of nine patients, on which ACI of the femoral condyle was performed, were studied 6 to 36 month postoperatively. MR imaging was performed with a 1.5T magnet before and 2 hours after intravenous administration of Gd-DTPA. R1 relaxation rate measurements were made with an inversion recovery sequence. Concentration of Gd-DTPA in the cartilage was calculated by the difference between R1 before and after administration of Gd-DTPA.\nSecond arthroscopy was performed 12 months postoperatively, and biopsies were taken from the center of reparative cartilage. The histology of reparative cartilage samples was compared with concentration of Gd-DTPA penetrated into the reparative cartilage.\n\\n(Results)\nThe concentration of Gd-DTPA in the reparative cartilage was higher than that in native cartilage, and there was significant difference (p\u003c0.05). This represents that the concentration of GAG in reparative cartilage was lower than that in native cartilage.\nOf reparative cartilages obtained from 8 cases, the proportion of hyaline cartilage in one case was only 20 %.\nIn latter case, most of the tissue filling the reparative cartilage was fibrocartilage, and the concentration of Gd-DTPA penetrated into the reparative cartilage was higher than that in the other cases.\n\\n(Conclusion)\nThe evalution of GAG concentration in the reparative cartilage was possible by using the Gd-DTPA-enhanced MR imaging. This method may be very useful for non-invasive evalution of the maturing prosess in transplanted cartilage and the long-term durabiliry of reparative cartilage.
