内容記述 | [Objective] As represented in ‘punch-drunk’ syndrome of boxers, memory impairment is a common complaint following repetitive mild traumatic brain injury (rmTBI) in retied athletes. Previous pathological studies reported the severe mammillary bodies (MB) atrophy in chronic traumatic encephalopathy, and it might be associated with memory function. Then, we evaluated the relationship between MB atrophy and memory impairment in rmTBI athletes using MRI. [Material and Methods] For 23 rmTBI athletes (15 boxers, 3 kick boxers, 2 wrestlers and three others) and 17 age/sex-matched healthy controls (HC), we used brain tissue segmentation based on MP2RAGE images using 3T MRI. The volume of subcortical structures (MB, hippocampi, amygdaloid nuclei, caudate nuclei, thalami, corpus callosum) was evaluated using ITK-snap, and total brain volume (TBV) using SPM12. Mini-Mental State Examination, Logical Memory and Digit Span tests were carried for rmTBI athletes. TBV and the ratios (subcortical structures/TBV) were compared between rmTBI athletes and HC using Mann-Whitney U test and measured of the relationship between memory tests using Spearman’s rank correlation coefficient. [Results] There was no significant difference in TBV between rmTBI athletes and HC (1.11±0.13 vs. 1.17±0.13 ×105 mm3, p=0.143). MB/TBV ratio was significantly smaller in rmTBI athletes (159.3±57.7×10⁴) than that in HC (233.1±32.3×10⁴, p<0.001), but there is no difference in the other ratios. MB/TBV ratio was positively correlated with Logical Memory test (delayed recall) in rmTBI athletes (γ=0.469, p<0.024). [Conclusions] MB atrophy is related to memory impairment in rmTBI athletes, and might be reflected to the disconnection of MB from the Papez circuit. |
