内容記述 ・Aims and Objectives:

CT and MR images are recoded on CDs in accordance with the Portable Data for Imaging (PDI) profile developed by the Integrating Healthcare Enterprise (IHE) and distributed to patients. The Portable Data for Imaging Integration Profile specifies actors and transactions that allow users to distribute imaging related information on interchange media. The intent of this profile is 125 to provide reliable interchange of evidence objects and diagnostic reports for import, display or print by a receiving actor [1].

When image-based information is exchanged between health care institutions, it is often difficult to determine whether or not: an image is the original or a copy, it is appropriate to use the image in clinical practice, and its validity has been established.
The aim of the present study was to improve the recording of image-based information and exposure levels using DICOM PS 3.15 (Security and System Management Profiles) as part of quality management.

The study proposed establishing a system in which the status and evidence of image processing are recorded on the DICOM tags during the creation of PDI - technology to manage the quality of image-based information.
\n・Methods

1) The image-processing status at our medical institution was defined using
DICOM PS 3.15 .

2) Following this, the function to record the defined status on the DICOM Tag
SOP instance status (0100, 0410) of shot images was implemented.

3) In addition, the function to transfer information on the history of image-
processing to the Original Attribute Sequence (0400, 0561) at the time of
PDI output was also implemented.
\n・Results
1)Actor
 We defined each system as an actor. Details of the defined actor are shown in Fig2.
2) Status
\n The status was defined as the Authorized Original (AO) or a definitive object to be stored digitally. A copy of the AO, which has a quality similar to that of the original and is created when it is imported to other facilities, was defined as the Authorized Copy (AC). (Fig.3)
\n3) Rewriting and editing of DICOM tags
When an image was stored in the PACS, the following three tags were recorded on the image using the Evidence Creator: Authorization Date and Time (0100, 0420), SOP Authorization Comment (0100, 0424), and Authorization Equipment Certification Number (0100,0426)”. (Fig.4)
In addition, information on the three tags was transferred to the Original Attribute Sequence (0100, 0561) during the creation of PDI. (Fig.5,Fig6)
\n・Conclusion:
In the present study, the status of images was defined using DICOM PS 3.15. The actions required to determine whether or not an image is optimal, or who (and at what point) is responsible for its creation, were also defined digitally. Radiological technologists must judge the accuracy, reliability.

This study was able to establish workflows using this function, which improved their awareness of the optimization of images. The medical institution that receives the image has the possibility of rewriting it in the incorrect information. For example, it is an examining date, a modality type, and medical exposure dose information, etc. When the examining image of other medical institutions is stored from PDI to PACS, these information is very important.

Particularly, the medical exposure dose is an term thought to be the most important in the field of a safe medical treatment.

The handling of the medical exposure dose using the DICOM tag will be studied in the future.
