内容記述 Purpose: Carbon ion radiotherapy showed promising results in patients with skull base tumors. However, the occurrence of brain injury due to carbon ion radiotherapy may reduce their quality of life. Thus, the purpose of this study is to determine the risk factors for brain injury after carbon ion radiotherapy for skull base tumors.
Materials and Methods: Between April 1997 and January 2009, carbon ion radiotherapy at a total dose of 48.0–60.8 GyE was administered in 16 fractions over 4 weeks to 47 patients with skull base tumors. Of these 47 patients, 38 who were followed up for more than 24 months by magnetic resonance imaging (MRI) were included in this study. Histological analysis revealed that 24 patients had chordoma, 5 had chondrosarcoma, 4 had olfactory neuroblastoma, 4 had malignant meningioma, and 1 had giant cell tumor . Brain injury on MRI was assessed according to the Late Effects of Normal Tissue-Subjective, Objective, Management, Analytic criteria, and clinical symptoms according to EORTC & RTOG tables. The correlations of the clinical and dosimetric parameters with the incidence of >grade 2 brain injury were retrospectively analyzed using MRI.
Results: The median follow-up period was 67 months (range: 24&#8211;152 months). The 5-year actuarial likelihoods of >grade 2 brain injury and >grade 2 clinical symptoms were 24.1% and 5.7%, respectively. Of the 15 patients with >grade 2 brain injury, 3 developed >grade 2 clinical symptoms. No patient with <grade 1 brain injury developed >grade 2 clinical symptoms. Dose-volume histogram analysis revealed that the percentages of brain volume receiving more than 20, 30, 40, and 50 GyE (V50 ) were significantly higher in patients with >grade 2 brain injury than in those with <grade 1 brain injury. On univariate analysis, V40 and V50 were found to be correlated with the development of >grade 2 brain injury. Multivariate analysis demonstrated that V50 was a significant predictor of >grade 2 brain injury (p = 0.006).
Conclusions: V50 was a significant risk factor for >grade 2 brain injury induced by carbon ion radiotherapy administered in 16 fractions over 4 weeks. However, the incidence of such injury is within acceptable limits, making carbon ion radiotherapy feasible for skull base tumors.
