内容記述 | Purpose: To evaluate the efficacy and complication of carbon ion radiotherapy in treating patients affected by lymph node recurrence from resected colorectal cancer. Methods and Materials: Between December 2004 and December 2009, a total of 26 patients with 34 lymph node recurrences from resected colorectal cancer were treated with carbon ion radiotherapy. Lymph node recurrence from colorectal cancer without distant metastasis was confirmed by computed tomography, magnetic resonance, and carbon-11 methionine positron emission tomography findings. The subsite distribution was paraaortic area, 59% (n=20); iliac, 26% (n=9); mediastinal, 9% (n=3); inguinal, 3% (n=1); and aortoiliac, 3% (n=1). Twenty-one of 26 patients had undergone multimodality therapy for locoregional and/or metastatic recurrence. The median age was 55 years (range, 38-85). The doses were escalated from 48GyE to 52.8GyE and were delivered in 12 fractions/3weeks. The median follow-up period was 16.8 months. Results: All patients completed the scheduled treatment course. No patient experienced acute or late morbidity of grade 3 or higher. Tumor response was evaluated in 34 lesions. Complete response (CR) was observed in 14 lesions and partial response (PR) in 13. Two lesions remained stable disease. The overall tumor response rate (CR+PR) was 79%. Remarkable anti-tumor effects were observed. The local control rates were 91% and 72% at 1 year and at 3 years, respectively. There is no local recurrence in the lesions receiving 50.4GyE or more. The overall survival rates were 90% and 62% at 1 year and at 3 years, respectively. Conclusions: Carbon ion radiotherapy seems to be a safe and effective modality in the management of lymph node recurrence from resected colorectal cancer, providing good local control and offering a survival advantage without unacceptable morbidity. |
