内容記述 The Kana (Japanese syllabogram) Pick-out Test, treated as inspecting higher-order brain
function, has been proposed to be a suitable test for screening persons with mild or slight dementia and widely used in public health nurses in Japan. This test is a paper-based neuropsychological examination asked to pick out 5 kana letters (A, I, U, E and O; Japanese vowels) with a pencil to circle as many as possible within time limits while reading a short story written in kana letters. After the trial picking 5 vowel letters, the substance of the story is asked. Subjects are required to execute two tasks simultaneously: finding the 5 vowel letters and recognizing the substance of the story. By this test, subject's cognitive status was assessed, based on the number of 5 vowel letters selected correctly and retention of the substance of the story; prefrontal functions estimated as senility index. From the previous data of the statistical processing over 20,000 subjects in the neuropsychological investigation, the scoring results of the test show linear decline with aging after early 30 years old. However, in spite of such statistical accounts from numerous subjects, presumably because of technical difficulties, little is known about the experiments using fMRI while demonstrating the test. We therefore examined the effect of brain regional activity with computerized Kana Pick-out Test projected on the screen with clicking a mouse button to pick 5 kana vowel letters out under fMRI running. A total of 10 healthy right-handed subjects (7 males and 3 females, age 20 to 33 yrs, mean age 24.0 yrs) participated to the subjects. Executing the test resulted in significant increases in blood oxygenation level-dependent (BOLD) signals in prefrontal area, parietal association area, Wernicke's area and hippocampus. This signal intensity in prefrontal area with Kana Pick-out Test was stronger than that with only reading and retaining the substance of the story without picking 5 vowel letters out. These results indicate the existence of the attention pathway from and/or to prefrontal area as association mechanisms for execution of Kana Pick-out Test, suggesting that this test is useful in screening dementia.
