内容記述 Purpose: To describe treatment planning techniques for two-port carbon ion therapy of ocular melanoma which was recently started with the intent to reduce incidence of radiation complications further than previously undertaken mono-port therapy since 2001.
Methods: We have used in house developed EyePlan for mono-port treatment, however its modification was impractical. In order to start two-port treatment sooner, an available treatment planning system (TPS), XiO-carbon, CMS Inc., (1) was tested if applicable. We evaluated that its precisions of geometrical tools and dose calculations are acceptable except for DRR calculations. Thus we adopted the TPS for two-port treatment.
A nozzle was prepared for use of horizontal beams. The details are reported separately in this meeting. Carbon ion broad beams with spread out Bragg peaks are used for irradiation. Beams are shaped using ridge filters (RF), apertures and range compensators. RFs were designed so as to deliver dose to cause uniform HSG cell kill within SOBP. Patient CT scans were taken at treatment position. Dose distributions are calculated with broad beam algorism using parameters determined by measurements and calculations. Precise DRR images for patient positioning are calculated using in house developed software (2).
Results: This TPS has been used for ten patients since October 2005. Among them, seven patients were treated with two ports due to lower dose to OAR; two fractions of vertical beams and three fractions of horizontal beams.
Conclusions: Treatment planning of ocular melanoma patients using a 3D conformal TPS has been acceptable for two-port therapy. However more geometrical resolutions are preferable in some of the software tools. Commonly used tools such as eye model display and image fusions of CT, MRI and fundus photograph are desirable and to be developed.
