内容記述 Purpose
The first aim of this presentation is to provide some introductory knowledge for charged particle therapy such as proton and carbon ion therapy to medical engineers and medical physicists who are familiar with the radiation therapy using X-rays, electrons, but unfamiliar with the charged particle therapy using protons and carbon ions. The second purpose is to show some examples of charged particle therapy facilities and the distributions of facilities in Japan. Final goal is making some discussions on evaluation of impacts of charged particle therapy from different viewpoints.
\nMethods
After tracing the history of the charged particle therapy briefly, we concentrate on the rationale of the radiotherapy including charged particle therapy, that is, the delivery of a high dose of energy to the tumor while sparing normal tissue around it. In addition we discuss issues related to interactions between charged particle beams and tissues such as linear energy transfer (LET), relative biological effectiveness (RBE) and oxygen enhancement ratio (OER). These properties characterize the charged particle therapy, especially, RBE dose carbon ion therapy.
Secondarily, we consider the status of charged particle therapy, especially in Japan. In Japan we have many proton therapy facilities and carbon ones and there are many plans to build another particle therapy facility by both governments and private hospitals. We compared the clinical index between X-ray radiotherapy and charged particle therapy from different viewpoint such as building costs, maintenance costs, required human resources and so on.
\nResults
In principle, charged particle therapy can realize relatively easily the rationale of the radiotherapy because high energy protons and carbon ions lose their energy gradually along the penetration into patients and the maximum energies beyond their ranges, which is observed as, what is called Bragg curve, the depth dose distribution by charged particles. Even if using IMRT or VMAT, it is not easy to spare the normal tissue without any low dose, because these techniques can only perfuse the total dose after all.
It is not easy to evaluate how much impact on radiotherapy proton therapy and carbon ions therapy make, by just the simple measurement. The evaluation depends on many factors, for example, that depends on the kind of insurance system, whether national insurance or private insurance. The costs of building charged particle facilities and maintenance costs are higher than X-ray radiotherapy system. However, the charged particle facility usually includes PET, CT, MRI and so on, therefore if we compare the just the therapy system itself, the costs of charged particle system become reasonable compared to X-ray therapy system.
The human resources required for charged particle therapy is another important issue. In this viewpoint, medical physicists are required in common for QA/QC for complicated system such as IMRT, VMAT and charged particle therapy system.
