内容記述 Objectives: MR imaging is more suitable than CT for evaluating local and nodal status for head and neck cancer. Thus, fusion imaging of MR and PET may be PET may be helpful, but its clinical implication has not been fully investigated. The purpose of this study was to assess the clinical value of image fusion of MR with PET images in head and neck cancer patients.
Methods: From July 2005 to June 2006, 62 consecutive patients underwent FDG-PET and MR scans. The interval between MR and PET was less than 22 days. Among them, 45 patients were freshly diagnosed or suspected of having head and neck cancer, 15 patients were during the follow-up period after surgery, and the remaining 2 patients had cervical lymph node metastases from unknown primaries. At first, MR images were interpreted, and then fused images of MRI and PET (MR/PET) were evaluated by two readers in a blind manner. The diagnostic performance was compared based on histopathological findings or at least 6-month clinical follow-up.
Results: For initial staging in 45 patients, 42 patients had histologically-proven malignant tumors. Sensitivity of MR alone and MR/PET interpretations for primary tumor was 98% and 100%, respectively. As to the lymph node metastasis, sensitivity and specificity were 80% and 95%, respectively, for both methods. Of these 45 patients, the diagnostic discrepancies occurred only in 4 patients (9%), 3 of which were accurately diagnosed by MR/PET. ON the other hand, of 15 patients with suspected recurrence, recurrent tumors were confirmed histologically in 11 patients, second malignant tumors were detected in 3 patients, and no recurrence was confirmed in one patient. Sensitivity of MR alone was 69%, whereas that of MR/PET was 93%. The diagnostic discrepancies occurred in 4 patients (27%), all of which were accurately diagnosed by MR/PET. In 2 patients with unknown origin, primary site was not detected in one patient, while a tonsil cancer was identified as a primary only by MR/PET.
Conclusions: The fusion imaging of MR and PET images might be useful in evaluating head and neck cancer, especially for suspected recurrent cases rather than fresh cases in head and neck cancer patients.
