内容記述 Brachytherapy for prostate cancer with 125I seeds is a well established procedure, and the number of this procedure has been increasing in Japan. While it was reported that this procedure showed high effectiveness, concerns arose over the radiation doses and safety for medical staff because medical staff, in particular, a surgeon would stand close to the source during this procedure. To ensure safety for medical staff, we measured surface doses to medical personnel during the 125I brachytherapy.
Dose measurements were performed with photoluminescent glass dosimeters (PLDs) and thermoluminescence dosimeters (TLDs). PLDs have the advantage of the large measurement dose range. PLDs are easy to handle in clinical environment since they can be read repeatedly. The PLD system consisted of silver-activated PLDs and a PL reader, and used pulsed UV laser stimulation. PLDs were used to measure surface doses to a surgeon and an assistant for 20 and 17 times, respectively. TLDs were used to check the accuracy of the measurement and to measure surface doses to a nurse, a radiologist, an anesthesiologist and a radiological technologist. These dosimeters were calibrated with a traceable ionization chamber for photon beam at the corresponding energy. The variation of sensitivity for PLDs and TLDs for the effective energy of 28-37 keV was estimated to be about 9% and 11%, respectively.
PLD readings agreed with those of TLDs within about 10% for doses above 100micro-Gy outside of a few exceptions. Surface doses at the surgeon's left arms and hand reached up to 509micro-Gy and 650micro-Gy, respectively. Doses at other monitoring points of a surgeon and at all points of an assistant were below 100micro-Gy. The doses at surgeon's hands in the lead gloves of 0.03mm Pb nominal thickness were 50-70% lower than those at arms out of gloves. The tube voltage, tube current and irradiation time in fluoroscopy decreased gradually due to the increase in the surgeon's experience. Doses at all monitoring points of a nurse, a radiologist, an anesthesiologist and a radiological technologist were below 10micro-Gy since they positioned away from the source, and they were shielded by a protective lead glass.
