内容記述 PURPOSE: In this study, we evaluated the usefulness of MET-PET for pre-operative assessment of head and neck cancer patients. We compared PET findings to pathological findings of specimen from surgical operation for this purpose.
\nMETHOD AND MATERIALS: We studied 13 patients (8 males and 5 females) with proven head
and neck cancer who had been referred to our hospital for surgical operation. The average patient age was 61.4 years (range 55 to 78). The histological type were squamous cell carcinoma, adenocarcinoma, carcinoma ex pleomorphic adenoma and Warthin
tumor. We evaluated MET uptake of the lesion semi-quantitatively on the basis of the Tumor-to-Normal-tissue Ratio (TNR).
\nRESULTS: All lesions were clearly demonstrated by MET-PET in our cases. TNR of each cancer was 7.4(range 4.7 to 10.1), 5.2, 8.1, 3.3, 4.2, 0.5 and 4.8 in squamous cell carcinoma, adenocarcinoma, carcinoma ex pleomorphic adenoma, mucoepidermoid carcinoma, Warthin tumor, hemangioma and osteomyelitis of the mandible, respectively. The significant difference of TNR was shown between benign and malignant tumor (p=0.032). We classified our malignant tumors into two groups
such as group of squamous cell carcinoma and group of other carcinoma (adenocarcinoma, carcinoma ex pleomorphic adenoma and mucoepidermoid carcinoma), but the significant difference of TNR was not shown between the two groups (p=0.76). We also classified squamous cell carcinoma into two groups according to degree of tissue differentiation such as well differentiation group and others (middle and poorly differentiation), but the significant difference of TNR was not shown between them (p=0.15).
\nCONCLUSIONS: MET-PET successfully detected head and neck cancers as high accumulation lesion in our cases, and it though that MET-PET might be very reliable technique for head and neck cancer assessment. Because of limited number of cases of
this study, there was no statistical significant difference about pathological grouping and tissue differentiation of squamous cell carcinoma. We hope that sufficient number of cases might show the statistical significant difference about them.
