| 内容記述 | Background;Selective serotonin reuptake inhibitors(SSRIs)including paroxetine have indication for panic disorder. Reverse tolerance, or sensitizationi, is the phenomenon that increase effiectiveness with continued using of single drugs which is often seen in psychostimulants. But this seldom is reprted wish SSRIs. Method;Case studies of the reverse tolerance for paroxetine and non-systematic review of literature. Results;Two cases with panic disorder are presente. The first is 35years old male and the second is 33 years ood female. Both of them were successfully treated by paroxetine upto 30mg per day. They abruptly stopped their medication without medical control luckily enough without any discontinuation syndrome of SSRIs. After about one year fo asymptomatic period, their panic symptom were recurred. The frequency of panic attacks were incleasing day by day, and reached finally three times a day. Then they took 20mg of paroxetine again which resolved the attach within 10days. After several months of medication, they again voluntarily stopped taking the pill and the symptom recurredrecurred whthin one year. In this time, only 10mg of paroxetine induced them complete remission within three to four days. With literature search, only one desreption was found which indicated the reverse tolerance of side effect with paroxetine. Conclusion;Reverse tolerance for paroxetine has seldom reported. This phenomenon has seen for both therapeutic effect and side effect. On restarting this drug, clinicians must be caurtous for this phenomenon to avoid overmedication. |
