内容記述 | To estimate the toxicity and efficacy of a phase I/II or phase II clinical trial for patients with central nervous system (CNS) tumor treated with carbon ion radiotherapy. One of 2 phase I/II clinical trial for brain tumor was carried out from October 1994 to February 2002, and the diseases treated were low-grade astrocytoma (WHO grade 2), malignant glioma, and metastatic brain tumor. Another phase I/II clinical traial for malignant glioma was carried out from April 2002 and August 2008. A phase I/II dose escalation study for skull base and paracervical tumor was initiated in April 1997. The phase I/II dose escalation trial was performed up to the fourth-stage dose level. From April 2004, a phase II clinical trial for skull base and paracervical tumors was initiated under the Highly Advanced Medical Technology scheme with an irradiation schedule of 60.8 GyE in 16 fractions over four weeks. At the time of analysis, there was no evidence of any serious acute or late reactions in brain and skull base and paracervical tumors. In the treatment of low-grade astrocytomas with carbon ion radiotherapy alone, the carbon ion dose was escalated to the second stage. There were distinct improvements in PFS and overall survival (OS) in the high-dose group. The result of Combined X-ray radiotherapy, chemotherapy, and carbon ion radiotherapy for malignant gliomas indicate that the survival rate tends to improve with increases in carbon ion dose. For skull base and paracervical tumor, the carbon ion dose in excess of 57.6 GyE improves local control. |
