内容記述 | Recently, digital mammography units have been introduced into clinical use in Japan and patients undergoing digital mammography have been increasing. Because breast is relatively sensitive to radiation among organs, the evaluation of average glandular dose (AGD) in digital mammography is important for the optimization of exposure parameters and the assessment of the risk to radiation-induced cancer. The aim of this study is to evaluate the AGD for digital mammography performed in several medical facilities and to suggest the method of reducing doses with maintaining diagnostic image quality. We measured the exposure doses at entrance surface of the each phantom using four types of digital mammographic units and two types of phantoms consisting of 50% glandular and 50% adipose tissue, and evaluated the AGD per image from the product of the entrance surface doses and the conversion factor for glandular tissue. The AGD evaluated with the RMI 156 phantom were 1.7 - 2.1 mGy for the tube voltage of 28kV, and the target/filter combination of Mo/Mo, which were slightly higher than those in screen/film mammography. For breast thickness of 3.8 cm corresponding to mean compressed breast thickness of Japanese patients, the AGD evaluated with the BR-12 phantom were 1.2 – 1.6mGy. The wide variation of doses among digital mammographic units will cause the difference of the x-ray beam energy and mAs determined by the auto exposure control system. The AGD for Japanese patients in digital mammography were slightly higher than those in conventional screen/film mammography because Mo/Mo was still mainly used in digital mammography and mAs selected in digital systems was higher than that in screen/film systems. The AGD for Japanese patients can also be reduced by the use of harder x-ray beam and smaller mAs without affecting diagnostic image quality. | |||||
