One is the localized dose distribution just on a tumor in a human body. It comes from the Bragg peak in the energy deposition and the low multiple scattering of the heavy-ion beam. The other one is the large relative biological effectiveness (RBE) due to the high linear energy transfer. The carbon ion is one of most effective species due to RBE. Over 3000 cancer patients have already been treated with 140-400 MeV/u carbon beams by the Heavy Ion Medical Accelerator in Chiba (HIMAC) at the National Institute of Radiological Sciences (NIRS) since 1994. These clinical results have clearly verified the advantages of carbon ion. Based on our experience at HIMAC, a hospital-specified facility optimized for the carbon-ion therapy has been designed. Its area size and construction cost are almost 1/3 of HIMAC, respectively. The prototype developments of an ECR ion source, a RFQ linac, an IH linac, an acceleration system of synchrotron, a beam-delivery system, and other key-technology parts have been successfully finished. Thus, in co-operation with NIRS, Gunma University has been constructing the carbon-therapy facility since April 2006. There are many other construction plans in the world. In order to improve the present clinical results, it is necessary to create more accurate dose distribution on the tumor without undesired dose deposited on the normal tissue. Beam-scanning methods with respiration-gated irradiation are especially important to treat a cancer located in the trunk of patient. Developments on the new repainting raster-scanning method and so on are now in progress.
