内容記述 | Purpose: High control rates and minimal adverse reactions were reported in National Institute of Radiological Sciences, Chiba, Japan for the stage I non-small cell lung cancer (NSCLC) with carbon ion radiotherapy (CIRT). We describe our current planning methods in CIRT for the patients with NSCLC located in the peripheral or hilar regions. Methods: The NSCLC on the peripheral zone of the lung was treated from 4-portal oblique irradiation directions with single fractionation. In contrast, the central type of the NSCLC was irradiated with total dose 68.4GyE in 12 fractions. We define the two targets such as CTV-1 included the bronchus near the tumor and CTV-2 excluded the bronchus near the tumor as much as possible. We irradiated CTV-1 and CTV-2 with total dose 51.3GyE/9 fractions and total dose 17.1GyE/3fractions each other. All of patients treated with these planning were enrolled to the clinical trials. Results: We did not experience severe adverse reaction to date in these protocol studies. We estimate the significant effect of these treatments from conventional progress. We reveal the relation between PTV and the respiratory movement of the tumor by the four-dimensional CT (4D-CT) under free breathing in several cases of the peripheral type. We compare the findings by the periodic bronchosopy with dose distribution on the inside of the bronchus by the virtual bronchoscopy in the hilar lung cancer. Conclusions: We performed the each different treatment planning of patients with the NSCLC. We have to treat more patients to demonstrate the effectiveness and impact of these planning by the 4DCT or by the dose distribution on the virtual bronchoscopy. |
