内容記述 Photodynamic therapy (PDT) is a method to treat tumors utilizing photodynamic reactions between photosensitive substances with tumor affinity and lasers. For overall bile duct cancers, PDT shows an effect to resolve stenosis by cancer and improves the prognosis, but when it is limited to intrahepatic bile duct cancers, some modification of laser irradiation is necessary because surrounding hepatocytes incorporate a large amount of photosensitive substances. Moreover, the intrahepatic bile duct is thin and a guide sheath and thin fiber are necessary to transport a probe for laser irradiation to the target region. To target a thin intrahepatic bile duct, we developed a parallel-type ultra-small composite optical fiberscope (COF) with an outer diameter of 1 mm or smaller, and performed PDT in an animal model using talaporfin sodium (product name: Laserphyrin), which cells (hepatocytes) rapidly excrete and is applicable with a long-wavelength laser having high tissue penetrating ability. We confirmed that Laserphyrin does not cause necrotic changes in the normal biliary tract mucosa. In addition, COF images with a certain quality could be acquired.
