| 内容記述 | Progressive Supranuclear Palsy (PSP) is a neurodegenerative disorder characterized by impaired movement and frontal cognitive dysfunction. Although motor symptoms have been associated with subcortical tau deposits, the network mechanisms underlying frontal cognitive symptoms remain unclear, because pathological tau accumulation is quite limited in the frontal cortex and instead is heterogeneously distributed mainly within subcortical regions. To tackle this gap, we combined high-contrast florzolotau-PET with a normative connectome approach for 37 patients with PSP-Richardson syndrome. To map the functional network associated with these tau deposits, we applied a tau-network mapping approach based on lesion-network mapping. Notably, this method does not require rs-fMRI data from the patients themselves, but instead leverages a large open database of rs-fMRI from healthy individuals. Resting-state functional connectivity maps were computed using rs-fMRI data from 100 healthy participants, treating each partient's tau-deposit VOIs as seeds. Tau-deposition sites were connected to a common cortical network across the patients that could not be detected when similar analyses were performed using atrophy instead of tau seeds. This network overlapped primarily with canonical action-mode and frontoparietal networks. Critically, whereas the degree of subcortical primary tau deposition correlated with motor symptoms, only the strength of connectivity from tau-deposition sites to the derived cortical network explained the frontal cognitive deficits of individual patients. These findings suggest a novel mechanism in which frontal cognitive impairments, but not motor deficits, in PSP arise from remote network effects of tau deposits, independent of atrophy, through convergent connectivity to a common cortical network. | |||||||
