内容記述 Japanese guidelines recommend surgery or chemoradiotherapy (CRT) for submucosal (T1b) esophageal cancer. Approximately 44.5% of all patients with esophageal cancer are aged ?70 years,1 and hence, surgery and chemotherapy may be unsuitable owing to comorbidities associated with old age. Therefore, less-invasive and effective treatments are needed for the management of esophageal cancer, especially in elderly patients.

Recently, carbon ion radiation therapy (CIRT), which has several physical and biologic advantages over conventional radiation therapy (RT), has been developed as a cancer treatment.2,3 In the first clinical trial of CIRT for esophageal cancer, neoadjuvant CIRT without chemotherapy was tested as a dose-escalation study, and a complete response (CR) was pathologically achieved in nearly 100% of T1 cases.4 Therefore, we aimed to examine whether CIRT alone could cure clinical T1b esophageal carcinoma. However, the dose of CIRT that the normal esophagus can tolerate is still unknown. In this article, we report the first results of a phase 1/2 clinical trial of CIRT to evaluate the safety and effectiveness of clinical T1bN0M0 esophageal squamous cell carcinoma (ESCC).
