However, objective neuroimaging patterns linking the history of rmTBI are still not established. We described the unique findings of conventional MRI and PET images in retied athletes with rmTBI. Material and Methods: We enrolled 25 retired athletes exposed to rmTBI (17 boxers, 3 kick boxers, 2 wrestlers and three others) and 18 age/sex-matched cognitively healthy controls. We evaluated the conventional MRIs (MPRAGE, T2WI, SWI) referred to the gross pathological findings of chronic traumatic encephalopathy (CTE). [18F]-PMPBB3 (tau-PET) and [11C]-PiB (amyloid-PET) were also evaluated. Results: In retied athletes with rmTBI, MPRAGE and T2WI showed enlargement of the lateral and third ventricles (both 36%, 9/25), cavum septum pellucidum (60%, 15/25) and enlarged perivascular spaces (56%,14/25). Atrophy of the mammillary body (68%, 17/25), thalamus (12%, 3/25), brainstem (24%, 6/25) and corpus callosum (40%, 10/25) were also characteristic findings, but the volume of hippocampus and amygdala were comparatively reserved. In SWI, two cases (8%) showed absence of “swallow tail sign” in midbrain related to parkinsonism. No cases showed intracerebral hemorrhage or contusion. Tau-PET images showed multi-focal or widespread uptake of the tracer in the depth of the neocortex and the white matter, but amyloid-PET images were negative in all cases. Conclusions: We showed the unique MRI and tau-PET findings in retied athletes with rmTBI, which may reflect the pathological change of CTE and can facilitate the differential diagnosis for radiologists.
