I-123-IMP SPECT (IMP SPECT) was reported to be a useful diagnostic imaging tool for choroidal melanoma. However, evaluation by SPECT is qualitative with inadequate anatomical information, and there are some issues to be left for the accuracy of its methodology. The purpose of this study was to assess the diagnostic ability of IMP SPECT and to assess semi-quantitative evaluation applying SUV in patients with choroidal melanoma. We also examined the association between SUV and uptake index (UI) which was a conventional method. [Methods] Twelve patients (5 males, 7 females) underwent IMP SPECT as an assessment before carbon-ion radiotherapy for choroidal melanoma. The mean age of patients is 62.4 ± 9.63 (ranged from 42 to 76) years old. SPECT imaging was performed using a gamma camera 6 and 24 hours after intravenous injection of 215.3 ± 8.4 MBq of 123I-IMP as the 2 phase-imaging. For data analysis by SUVmax, 6- and 24-hours SPECT images were fused with CT images using an image-analysis software. The volume of interests (VOIs) of the whole eyeballs on the tumor side and the contralateral normal side were drawn, followed by semi-quantitative evaluations by SUVmax. Uptake index (UI) by radioisotope counts on SPECT images, which is a conventional method, was also examined to compare with the diagnostic ability of SUVmax. [Results] Six hours after intravenous injection, SUVmax of tumor side was 2.37 ± 0.88, normal side was 1.77 ± 0.39, significantly higher on the tumor side (P <0.05). After 24 hours, SUVmax of tumor side was 3.25 ± 1.57, normal side was 1.87 ± 0.22, significantly higher on the tumor side (P <0.01). There was a high correlation between SUVmax after 6 hours and 24 hours on the tumor side (ρ = 0.84, P <0.001). Six hours after intravenous injection, the UI of tumor side was 2.24 ± 0.67, normal side was 1.53 ± 0.35, significantly higher on the tumor side (P <0.05). After 24 hours, the UI of tumor side was 4.03 ± 1.30, normal side was 1.95 ± 0.46, significantly higher on the tumor side (P <0.01). There was no significant correlation between tumor-UI and tumor side SUVmax after 6 hours (ρ = 0.47, P =0.124), whereas high correlation between those of after 24 hours (ρ = 0.87, P <0.001). [Conclusions] Semi-quantitative evaluation using SUV had an association with conventional UI.
