Objective: The aims of the present study were to evaluate a theoretical model of predictors of dementia-related caregiver burden, and to identify the pathological basis of BPSD causing caregiver burden. Material and Methods: Seventy-one patients, aged 53-97 years, with cognitive impairments were enrolled. Caregiver burden and BPSD were evaluated by Zarit burden interview (ZBI) and neuropsychiatric inventory (NPI), respectively. Twenty-nine of 71 patients, consisted of Alzheimer’s disease (AD) or mild cognitive impairment patients, underwent [11C]PiB-, [11C]PBB3-, and [18F]FDG- PET scans for evaluating brain regional amyloid-beta (Aβ), tau depositions and glucose metabolism. Path analysis was performed to identify BPSDs directly and indirectly associated with caregiver burden assessed by ZBI. We also examined correlations of regional Aβ, tau depositions and glucose metabolism with BPSDs related to caregiver burden. Results: Path analysis revealed that agitation/aggression (β=0.62) and anxiety (β=0.39) had positive direct effect on caregiver burden, and that irritability/lability, disinhibition and depression/dysphoria had positive indirect effect on caregiver burden (Figure). All patients underwent PET scans were PiB-positive, and there was no significant difference in Aβ, tau depositions and glucose metabolism between patients with or without the above-mentioned BPSDs; however, caregiver burden-related positive and negative BPSDs were significantly correlated with tau deposits in the frontal and fronto-parieto-temporal regions, respectively. Conclusions: Cortical tau deposition would formulate the pathological basis of BPSD, resulting in the increment of caregiver burden.
