[Materials and Methods] Thirty-three patients (mean age, 46.2 years) with bone and soft tissue sarcoma of the head and neck who treated by C-ion RT with 70.4 GyE/16 fractions (fr.) were enrolled into this study. The endpoints were acute and late reactions of normal tissues, the local control rate and the overall survival rate. Additionally, the efficacy of the treatment with high-dose was compared with our historical results using C-ion RT with 57.6 or 64.0 GyE/16 fr. [Results] The median follow-up period was 32.4 months. The 3-year local control rate and overall survival rate were 85.6% (95% confidence interval (CI) = 69.4-100%) and 75.2% (95% CI= 49.2-100%), respectively. In comparison with our historical results (n=14), the local control rate with 70.4 GyE was significantly better than thatwith 57.6 or 64.0 GyE (3-year, 85.6% vs. 21.4%, p<0.0001), and the dose escalation of C-ion RT tended to improve also the overall survival (3-year, 75.2% vs. 42.9%, p=0.06). A grade 3 acute reaction was observed in only 1 patient. With regard to late reactions, visual loss was observed in 1 patient, and Grade 3 toxicity of the maxillary bone was observed in 4 patients. [Conclusion] C-ion RT with 70.4 GyE/16fr. for bone and soft tissue sarcoma of the adult head and neck appears to be effective and has acceptable toxicity.
