Eleven ONs presented evidence of visual loss due to carbon ion radiotherapy. Prior to carbon ion radiotherapy, all patients gave written informed consent accepting the possibility that visual impairment might occur as a result of the treatment, and the institutional ethics committee approved the study. Before radiotherapy and in the post-treatment follow-up period, the patients were routinely submitted to magnetic resonance imaging (MRI) and clinical examinations by ophthalmologists. For the diagnosis of radiation-induced neuropathy of the ON, the following diagnostic criteria were used: no tumor invasion and contrast enhancement of the ON on MRI, prolongation of latency and/or attenuation of amplitude in visual evoked potential (VEP). The dose-complication probability and the prognostic factors for the retention of visual acuity in carbon ion radiotherapy were analyzed using dose-volume histograms (DVHs) and multivariate analysis. This result was published in 2006.\nMETHODS AND MATERIALS: From April 2000 to March 2003, a total of 116 patients with tumors in the head and neck or the skull base region were analyzed. Thirty-one patients of 116 patients had a follow-up period of more than 2 years and whose ONs had been irradiated more than 45 GyE. Thirty-four ONs corresponding to 31 patients were evaluated the dose of ON using DVHs. Additionally, these DVHs compared with previous data.\nRESULTS: The median age of 31 patients was 55 years with 19 males and 12 females. About a tumor site, the patients consisted of 17 maxillary sinus, 4 ethomoid sinus, 4 palate, 2 nasal cavity, and 4 others. The median maximum dose (Dmax) of the ON was 57 GyE. In the 34 ONs that were analyzed, 5 of the 23 ONs (22%) irradiated with more than 55 GyE (Dmax) suffered a decrease of visual acuity.\nCONCLUSION: This study has be evaluated the dose-complication probability for the impairment or retention of visual acuity.
