The purpose of this study is\nto evaluate the tolerance for and effectiveness of carbon ion radiotherapy\nin patients locally recurrent rectal cancer </b><br>\n<B>Materials/Methods: Between April 2001 and February 2006, 70 lesions at 65\npatients were enrolled onto this study. Criteria for trial eligibility were\nconfirmation of locally recurrent rectal cancers without distant metastases\nbased on CT, MRI and PET findings and ECOG performance score 0,1,2.\nContraindications for trial entry included pelvic bone destruction or\ninfiltration into the bladder. Carbon beams of 290, 350 and 400 MeV/nucleon\nenergy were generated in the HIMAC synchrotron. The dose was determined as\n67.2GyE and escalated to 70.4GyE,73.6GyE. Of the 65 eligible patients, 42\nwere male and 23 female. Median age was 62.5 years. The predominant sites of\nrelapse were 26 presacral , 17 lymph nodes, 16 perineal and 8 anastomosis.\nToxicities on organs were assessed according to the NCI-CTC and RTOG/EOTRC\nclassification. Tumor response was defined by the RESIST scoring system.\nLocal recurrence was defined in terms of lesions occurring in the tumor bed.\nSurvival curves were estimated by the Kaplan and Meier method.</b><br>\n<B>Results: Ten patients received radiation dose at 67.2GyE, 17(+1) at\n70.4GyE and 38(+4) at 73.6GyE. All toxicities in the 70 lesions at 65\npatients were relatively few and mild in these patients. No grade 3 to 5\nacute and late toxicity was observed. Tumor response was evaluated in 69\nlesions patients. CR was observed in 10 lesions and PR in 16. The local\ncontrol rates in 69 lesions are 93% at one year and 83% at two years. Local\ncontrol rates at one year were 80% at 67.2GyE, 86% at 70.4GyE and 100% at\n73.6GyE. In terms of symptomatic response within 3 months after treatment,\npain improved in 97% of the symptomatic cases. Pain relief was maintained at\none year in 67%, 91% and 100% of the patients treated with 67.2GyE ,,\n70.4GyE and 73.6GyE, respectively. The one and two year overall survival\nrate were 91% and 75% respectively.
