We evaluated whether or not early prediction of the occurrence of local recurrence and metastasis as well as prognosis are possible regarding patients with adenoid cystic carcinoma of the head and neck by PET or PET/CT using L-methyl-[11C]-methionine (MET). Methods: Sixty-seven cases that underwent MET-PET or PET/CT study prior to and approximately one month following the completion of carbon ion radiotherapy (CIRT) were prospectively investigated. MET accumulation of the tumor was evaluated using the semiquantitative tumor to normal tissue ratio (TNR). A univariate analysis and multivariate analysis were carried out regarding TNR of the tumor as well as several other clinical factors. Results: The average TNR prior to treatment and TNR following treatment was 4.8 (+/-1.5) and 3.0 (+/-1.3), respectively, showing a significant decline in the TNR following treatment. Upon univariate analysis, groups with a high TNR prior to treatment showed significantly more occurrence of distant metastasis compared to the group with a lower value, along with a poor prognosis. Groups with a high TNR following treatment showed significantly more occurrence of local recurrence compared to the group with a lower value. The cut-off values of TNR prior to treatment regarding distant metastasis and prognosis were both 5.6. The cut-off value of TNR following treatment was 3.5. Regarding the residual ratio of TNR changes of accumulation following treatment, there were significantly many occurrences of distant metastasis in the group with a low residual ratio, wherein the prognosis was poor; however, a significant negative correlation was observed between the residual ratio of accumulation and the TNR prior to treatment, with a tendency for the residual ratio of accumulation to be lower in cases with high TNR prior to treatment. In the outcome of multivariate analysis, TNR following treatment in addition to tumor size were the factors significantly influencing local recurrence. TNR prior to treatment, the residual ratio of TNR changes of accumulation, and tumor size were significant factors influencing the occurrence of distant metastasis. Regarding prognosis, TNR prior to treatment and age were factors with significant influence. That is to say, TNR prior to treatment was a significant factor regarding distant metastasis in both univariate analysis and multivariate analysis. In the same manner, TNR following treatment was a significant factor regarding local recurrence in both analyses. Conclusion: MET accumulation measured by PET or PET/CT was a factor significantly related to the occurrence of local recurrence and metastasis as well as early prognosis regarding CIRT for adenoid cystic carcinoma of the head and neck, thus suggesting it is useful for determining therapeutic efficacy., 平成23年度HIMAC共同利用研究成果発表会}, title = {頭頸部原発腺様嚢胞癌の重粒子線治療効果判定におけるメチオニン-PETの有用性:単変量および多変量解析を用いた検討}, year = {2012} }
