Expression of tenascin-C (TNC), one of the oligometric extracellular glycoproteins, is induced in myocardium during active stage of myocarditis. Therefore, TNC may be a molecular marker for disease activity in myocarditis. Methods: 111In-anti-TNC monoclonal antibody was injected intravenously to 10 experimental autoimmune myocarditis (EAM) rats and 10 control rats, and the biodistribution of this radiotracer was measured. Excised and sectioned hearts were stained with hematoxylin and eosin for histological analysis and were exposed to autoradiography. In vivo SPECT imaging was simultaneously performed with 111In-anti-TNC and 99mTc-Sestamibi. The SPECT system consisted of a 3-headed gamma-camera equipped with 1.0-mm pinhole collimators. Results: Average myocardial radioactivity was significantly higher in EAM rats than controls (2.39+/-0.17 %injected-dose/g vs. 0.32+/-0.03, p < 0.01). By autoradiography, high radioactivities were observed in the regions identical with inflammation in EAM rats. SPECT imaging in EAM rats clearly showed the focal myocardial uptake of 111In-anti-TNC, which is complementary to perfusion image. Conclusions: The present study indicated a high ability of anti-TNC antibody to localize sites of inflammation in the heart by ex-vivo and in-vivo imaging.
