Methods: A phase I/II clinical study of CIRT for ocular melanoma at NIRS was started in Jan. 2001, using 140MeV horizontal carbon ion beams. The methods of treatment planning and irradiation techniques were the same as the proton radiotherapy (PRT). The patient who had a large-sized tumor, such as more than 15mm in diameter or more than 5mm in height, or a tumor close to optic disc was eligible in this study. Relative biological effectiveness (RBE) was estimated with biological experiments and clinical results in CIRT for other tumors. The applied dose was gradually increased with a starting dose of 70.0GyE/5f. We analyzed the patients treated in this study, in respect of normal tissue morbidities and initial tumor response comparing with PRT. Results: So far, twenty-four patients entered the study and 11 patients with follow-up period of more than 6 months were analyzed. All patients were alive without any recurrence at present. Skin reactions after CIRT were comparable to those of PRT. Neovascular glaucoma was observed in 4 patients and the incidence of glaucoma was strongly depending on the site and size of the tumor. For the initial response of the tumor, the decline of tumor height in CIRT seemed to be more definite than in PRT.
