We propose local myocardial ablation method targeted to severe ischemic regions in order to save the remaining jeopardized regions by heavy ion beam-irradiation. Heavy ion beam being used for a less invasive cancer therapy can be localized at the target tissue in x-, y-, and z-directions with 1 mm order accuracy. Six rabbits were irradiated through the left chest (20x20 mm) by a single dose of heavy ion beam (290 MeV/u carbon-12, 92 Gy). The irradiation could be focused to the subepicardial region of the anterior left ventricle wall and did not reach to the subendocardial region. Four weeks after the irradiation, histological necrosis was confined in the outer one-third of the irradiated region. Autoradiographic analysis with 125I-metaiodobenzylguanidine revealed that transmural denervation (>85% decrease of 125I-uptake) was noted in the irradiated region but not in any other part of the heart. Contractile function evaluated by echocardiography (%FS: 27.2+5.8 vs 28.7+3.7%, n=6) and regional myocardial blood flow by microsphere method (1.76+0.56 vs 1.84+0.59 ml/min/g, n=6) were not different between the irradiated and the non-irradiated regions. General conditions and blood indices of bone marrow-, liver-, renal-functions were well maintained until sacrifice. The heavy ion beam ablation might be a possible radiotherapy for refractory heart failure resulting from an extensive myocardial ischemia, because the method allows a less invasive ablation of the severely ischemic myocardium than the conventional therapies.}, title = {Localized myocardial ablation using heavy ion irradiation as a possible therapy of myocardial ischemia-induced refractory heart failure}, year = {2002} }
