This difficulty is typically related to the lesion's\noriginal location or the possibility that resection could lead\nto a marked decline in the patient's quality of life, based on\npostoperative motor dysfunction or neurological damage [1].\nMoreover, even in cases where resection is possible, postoperative\nirradiation is recommended for patients who experience incomplete\nresection or exhibit positive surgical margins. However,\nmany kinds of bone and soft-tissue sarcomas are radio-resistant\ntumours, and it is can be difficult to control them using X-ray radiation\ntherapy.\nIn the field of radiation oncology, heavy-ion beams are defined\nas radiation that is obtained by accelerating charged nuclei that are\nheavier than protons. Carbon ion radiotherapy is one type of heavyion\nradiotherapy having potential advantages over conventional\nradiotherapy [2]. For example, carbon ions provide better physical\ndose distribution, compared to conventional radiotherapy, because\nof the special characteristic of charged particle beams (the Bragg\npeak) [2,3]. Furthermore, they have a higher relative biological\neffectiveness and a lower oxygen enhancement ratio. Some researchers\nhave argued that protons represent a technical\nimprovement for highly conformal therapy, and that heavier ions\nmight even facilitate the treatment of cancers that are resistant to\nconventional X-ray therapy [4]. In Japan, the National Institute of\nRadiological Science (NIRS) has treated patients using carbon ion\nradiotherapy since 1994, and over 8000 patients with various solid\ntumours have been treated using by carbon ion radiotherapy at\nNIRS [2].\nWe informed two patients that there are not only advantages of\nsurgery but also postoperative complications. In addition, we told\nthem that carbon ion radiotherapy might be a safe and effective\nmodality for patients who localized primary sarcoma who are unsuitable\nfor resection or for patients who refuse surgery including\namputation. As a result, both of them made a choose carbon ion\ntherapy.\nIn the present report, we describe two cases of pelvic sarcomas\nwith available oncological and functional results from prolonged\nfollow-ups (>10 years) after carbon ion radiotherapy without surgery.\nInformed consent has been obtained from these two patients\nfor publication.
