One of the topics in this field is that dipyridamole- or adenosine-induced coronary flow reserve, as measured by positron emission tomography, is reduced in patients with traditional risk factors, such as hyperlipidaemia, hypertension, diabetes mellitus, and cigarette smoking, even in the myocardium supplied by angiographically normal coronary arteries. These studies demonstrated the early and diffuse impairment of coronary vasomotion in such patients. However, it is recognized that a large proportion of patients with coronary artery disease do not have any of the traditional risk factors. Several new risk factors, for example infection, may lead to coronary atherosclerosis. This raises the question, do coronary artery disease patients without traditional risk factors have early and diffuse impairment of coronary vasomotion similar to patients with risk factors. In the present study, we measured ipyridamole-induced coronary flow reserve using [13N] ammonia and positron\nemission tomography in the myocardium supplied by normal coronary arteries in patients with one-vessel coronary artery disease without traditional risk factors.
