To apply diffusion-weighted imaging (DWI) using echo-planar imaging with SENSE (sensitivity encoding) to prostate and evaluate whether prostate carcinoma can be demonstrated.
Two healthy volunteers and 15 patients with histologically proven prostate cancer were examined with 1.5T MR unit. Following routine axial and coronal T2 and axial T1-weighted images, DWI was obtained in the axial plane with a single-shot echo-planar-imaging (EPI) sequence using SENSE method. A b value was 700s/mm2. Diffusion-weighted gradients were applied in 6different directions in each of the measurements. ADC (apparent diffusion coefficient) map and Diffusion tesor imaging (DTI) were calculated with a computer program on numerical integration software. Vector-icon image were obtained to visualize the diffusion anisotropy of the prostate.
With ADC map, the lesion of prostate carcinoma was demonstrated as low intensity area and ADC map helped the localization of carcinoma. With DTI, a concentric anisotrophy was demonstrated in the normal prostate gland. The lesion of carcinoma deranged that anisotrophy. In two patient, low intensity area in ADC map was disappeared after HIMAC treatment. The ADC map and DTI have a potential to reveal prostate carcinoma and can be used for the evaluattion of the treatment.
